I identified the problem, and disabled the extension. The server is back online BUT without the broken extension a lot of features are not going to work. This includes Reserved slots and Map Chooser.

This is temporary solution.

Quick Update:

I believe the SM devs released updated gamedata but since the server is full for now, I'll work on the server later tonight when its empty.

Reserved slots and Map Chooser are currently not working.

The server is online and everything is back to normal.
Reserved slots and the Map Chooser are both working.

Until Valve releases a new patch and breaks everything again, that is. :)
